Thanks to whoever sent that in. I used to have it on paper somewhere but couldn't find it anymore. However, page 2 is missing the top part. Can the unknown contributor rescan that page, please?

While I did not send in this document...I do have a good copy of it...I can probably get it scanned for a lot less space than 30+ Megs too.

Film-Tech has a 1200dpi minimum for manual scans.
