Does anyone use an official performance evaluation when reviewing employees? If so what areas do you concetrate on? How do you rate them and how long does the evaluation last?

We have a standard evaluation form. Usually every employee gets evaluated after 90 days. After that, it depends on performance levels, but I think it is a good idea to evaluate entry people every 6 months to keep them on their toes.

Checkers always the best way for ground level employees. Give out passes to some freinds and have them fill out a sheet when they arrive. If the employee is going to screw around to a customer when he shouldnt be watching himself, thats the ultimate test. Most can smell performance evaluation time.
