I utilize theatre check cards here at my theatre. These small cards have edges on them with the theatre numbers, they also have spaces for the men's and women's rooms. We have individually shaped hole punchers in each auditorium. We use these for our 20 to 30 minute walk-thrus and our morning walks in the auditoriums. There is space (and the back) for extra notes concerning safety and maintenance items. They come in handy and I keep them on file for future reference - in case there is a legal issue. They really help when our mystery shoppers don't see the ushers on their auditorium checks.

I think Tim is looking for an auditorium condition report, not a method to ensure theatre checks.

Personally, I don't use any report, instead I just take pictures of any deficiencies with a digital camera and email them to whoever it concerns.

The old GCC projectionists site is still on-line. It had some forms, maintenance calendars, and logs:

What I was looking for is a form for a weekly walk-thru for maintenance/janitorial issues. But the forms that Dennis sent to me halps too. [Smile]
