How do they report these to the distributers?

Is this prevalent in the States?

Yes a chain in the United States tried to do this and there is a thread about it somewhere. The studios really hate this idea. I also think it limits your potential gross. We are doing quite well this year and we don't need to try this hard.

The Renters did try to block this loyalty card, but UGC still have this on offer and seams to be popular, there are some restrictions on some screenings on some new films (I belive)

I would guess the restrictions are on 'Free List Suspended' films. It's getting more common these days for the distributors to place a 'no free pass' clause into the booking contract for x number of weeks, in the case of Spidey, it's something ridiculous like 5-6 weeks from opening.
