Was this, up until now, a one-time thing or does it keep returning now and then?

This is probably just a communication error with the dolphin board: reseating this board and its ethernet cable is the first thing to do. Note the dolphin board has a cooling shroud that needs to be removed, also an extra mounting screw at the back of the card that has to be removed, and the card has the tamper detecting covering that will disable it if you handle it too roughly.
The motherboard or the dolphin board could be failing/defective.

If you already replaced the Dolphin card and you had EXCACTLY the same issues before, then it's most probably the motherboard that's faulty, like a problem with the PCI-x socket.

It could also be something else, like Dave already indicated: A problem with the internal Cat5e network connection, that cable is easily and cheaply replaced, so did you already try that?

Another thing that comes to mind is the seperate power connector that runs directly from the power supply via a standard Molex connector. There should be at least another free Molex power connector, you could try this as well.

The Security manager is that little board which is connected to the motherboard by the USB hub and an ethernet cable. I would check that ethernet cable and the connections around.

No. The SM is a software component, I believe it is in the Dolphin card. That little board is the boot flash drive.

Yes. The SM is the software running on the scb405 which is the part of the Dolphin card not covered by the black shell.

The card you mention is the USB hub, which happens to house the SSD drive.

From Doremi:
quote:
DOL12 (Dolphin DCI FIPS 1.2) and the SM (Security Manager also know as scb405), this picture shows the (top) the power connection to the scb405 and the (lower) is the RJ45 network connection, that goes to the USB hub.
