I need to know what model passive speaker combo is comprised of the following:

Horn: 2445J
Bass: 4648 (has 2 2225H Drivers)
Crossover: 3160

It can be one of two...

It can be the 4670 if the horn is small or the 4675 if the horn is large (does it overhang the cabinet in ALL directions).

Its a small rectangular horn that is flush with the 4648 cabinet.
